The decrease rows after you pick up stitches for the gussets are decreasing 2 stitches per row -- 1 stitch on needle one and 1 on needle three. If there are 81 stitches total (18, 16, 16, 31) using these decrease rows gets you to 61 stitches, not 60. We need one extra decrease so there are 29 stitches across needles 1 and 3, and 31 instep stitches on needle 2.
